If you are divorced or separated, please provide custody paperwork or court documentation that outlines who holds medical rights and decision-making authority.

Yes! School excuses are available—just stop by the front desk to pick up a form. If you forget, you can also call or email our staff to request one.

You’ll receive an email with a secure link to join your telehealth session at your scheduled appointment time.

Copays are collected at the time of your appointment.

Yes. To respect the time and commitment of our counselors, we ask that you provide at least 24 hours’ notice for any appointment cancellations or rescheduling requests.
Cancellations made within 24 hours of your scheduled appointment will be charged at your therapist’s private-pay rate for that session.
No-show appointments will be charged at your therapist’s private-pay rate for that session.
