Know Before You Go

To make the most out of your first appointment please fill out the required following items in the client portal:

Under the Forms & Documents tab, please complete the following:

  • Required for All Clients:

    • Credit Card on File Agreement

    • TeleMental Health Informed Consent

Then, go to the History tab and complete the questionnaire that applies to you:

  • Adults (18+) and Couples/Marital:

    • Adult Questionnaire (completed by the individual the appointment is scheduled under)

  • Teens (14-17):

    • Adolescent Questionnaire (completed by teen)

    • Parent/Guardian Questionnaire (completed by parent/guardian)

  • Kids (under 14):

    • Parent/Guardian Questionnaire (completed by parent/guardian)
